Case coordinator permanent positions

Community Health Centres provide a unique opportunity for case coordinators to:

  • Assess the nature of difficulty being experienced by the client with a comprehensive approach that includes identification of strengths and challenges, analysis of family situation and review of supports
  • Work effectively both independently and as a member of a team within an organization with a history of multi-disciplinary care
  • Work within a community-based context, with a culturally diverse community
  • Maintain and develop effective communication, interpersonal, organizational and administrative skills, including computer literacy
  • Be innovative in addressing individual, group and community health issues
  • Achieve a work/life balance (35 - 37.5 hours per week)
